Experienced this a couple of days ago and I did find at least two other players (on reddit) who reported the same thing.
Giving Bernard the correct code-phrase to obtain the ring did trigger the "NPC hands Tav an item"-animation while in dialogue mode, but when the dialogue ended, the ring was not in that character's inventory - and I didn't get an "item received"-pop up, either.
Double checked the party's inventory, but the ring wasn't there. Pick-pocketing seems to always fail on Bernard (at least for Shadowheart, lol), so I had to attack and kill him and all the animated armor-sets to loot the ring off his corpse.
I was kinda surprised that didn't break my Paladin's Oath, seeing how Bernard had just given me a nice robot-hug ...
