Prior to Patch 4, Hotfix 10/11, I've finished this quest twice with Duke Ravengard appearing where he should. I'm not sure if the issue is occurring because of Patch 4/the hotfixes, but this is my sequence for the Duke
to appear at the Iron Throne prison:
- Enter Wyrm's Rock. Do not go to the bridge, do not enter the lower city.
- Free Councilor Florrick from prison *I think this is optional, but you might as well since you're already here
- Enter the Audience Hall Chamber (ground floor, first door on your right). Mizora will be standing there waiting for you. I'm pretty sure if you enter this way, it's unmissable, and a forced cutscene/dialogue with Mizora will trigger. She will tell you to bring Wyll along to the Coronation.
- Go upstairs and do the Gortash coronation
- Once finished, go down to the bridge and enter the lower city/do what you want.
