Once I read "I'd recruited all Act 1 companions except Karlach" - everything you described here instantly makes sense. The root "cause" of your perceived issues is: you handled Minthara in a way that you unintentionally made all these happen. In the game, you are now officially an accomplice/follower of Minthara. However, everything will change once you make Minthara your follower.
