Well, what do you know, it was actually the passive not working.

Now, the oddities about it.
1- I didn't even know the weapon had a passive, since in the previous playthroughs I never turned it off.
2 The passive wasn't actually deselected. It was toggled as active but just not working. Toggling it off and then on again seems to have solved the issue.
