That happens when you cheat on someone.
In regards to never hitting and always dying: I started a new game just now and I have the impression that everyone hits less consistently early in the game than later on. I have even Lae'zel and Gale miss attacks and that just never happens later in the game. It's no surprise either because you have hardly any magical gear or other boosts to your stats so you have to be a bit more careful.
(On a side note to the side note: The harpies managed to charm my whole group yesterday and Lae trashed Gale before breaking free and taking bloody revenge on the bird ladies. It was a bit awesome but I almost thought, we were done for.)
