The same thing happened to me at Grymforge on the moving platforms in Act 1. It started moving, and two of my party members fell off into a chasm and died instantly. That was a close call. All of my characters were on that platform because I was using a mage hand to pull the lever. I agree it is a very obnoxious bug, and wouldn’t you know it, the only time I have ever encountered it was on honour mode!
