The last one on your list has been going on for a long time. The first one on the list I've experienced in my current co-op run since 21 as well. As a result that run has been entirely unplayable for us for almost two weeks.
I reported the bug right after 21 dropped and was told that it was a known bug with a fix in the works, only to see 22 drop without it being addressed. When I reported it a second time, I got the exact same form letter response email from the exact same support rep. This is the first time I've been genuinely disappointed by Larian's responsiveness.
