Just wanted to add a +1 to this, have been having the first, second, and last problems during a 2-player co-op honor mode run with the spouse.
Tried having a dialogue with Minthara in camp and got all three problems, making it incoherent and forcing a reload due to being stuck in dialogue. (A reload is a big annoyance in honor mode, as there is no quick save/quick reload. The game has to be shut down to main menu and restarted, and other players rejoin)
Seems like it's related to Hotfix 21 but we've been having the problem since we got into Act 3. It's bad enough that the spouse doesn't even want to finish the game.
I have a separate single player game and I haven't ran into this issue in single player.
