Could you confirm if after updating to Hotfix 23 the first issue on the list still persists?
Doing a little limited playtesting with two computers and didn't have this issue reappear. It's hard to know for sure since it seems to be somewhat random, but it didn't occur in ~30 minutes, including in the last place I had saved before encountering the problem and in running around through the grove initiating conversations with both party Tavs. We'll be able to put some actual multiplayer time in tonight, I'll update if I do run into it then!
