Turning Settings > Graphics > Animation Level of Detail to medium (was high) helped with the facial twitches on mine (M2 Pro, 16gb)
With it at high faces were very twitchy and Astarion's seemed the worst affected. The poor elf looked like he developed a severe facial tick from all the stress. Might be the same thing? Though I'd noticed that a long while back and don't remember which patch so if this is new it may be something else but worth a try. I also didn't notice map ripples.
I've also rolled back to Patch 6 (no hotfixes) using the instructions here which solved a lot of the current issues (FPS had dropped to unplayable levels with hotfixes past that for me):
