You can do that in DOS2 I believe as well as customize companions “class” when you meet them. I also seem to recall magic mirror allowed you to customize Origin appearance as well but maybe that was a Mod. Kind of surprising the way they set it up for BG3. Lots about Larian I don’t like but I have always appreciated the flexibility they generally try to offer.
