Act one, teahouse .
After cutscene when she transforms in a hag, she instantly dies withiout fight at all.
"auntie ethel succumbed to death" -written in the chat
This happens even if you just attack she in game - she instant die in hag form

On the Internet I did not find anything like this bug (

Sounds very similar to another problem I've noticed - when a Halsin is released from prison, it dies after a fight while returning to humanoid form from bear form.
It seems that shapeshifting/transform(?) is related to the bug/death.

Right now, encounter same bug with Halsin as you described

If you have a saved game state before these fights/stages you can save them for later, support will want you to submit them.

Happens with Shadow druids too, but tested auntie Ethel too. With mods or without them, it happens after them transforming. The scene breaks and NPC is dead. It is even worse for Kagha, since she becomes immortal NPC, game sees her as dead, but she is still around and will interact.

@Byoajeg, a question. Are you using any cheat engine tables? I just read that some cheat engine tables can cause this bug. Will try it on a new save and see if it works for me.

I used a cheat table for passive skills right before going into ethel's swamp: Even the redcaps ended up insta-dying. Not sure how cheat-engine messes with the game's memory, but I believe the game hiccups trying to find whom to give the passive skill to when something transforms. Then it kills whatever isnt in the player's party, hence the instadeaths.

Update as of writing this, I was sorta-right. People over on cheat engine forums are talking about temporary workarounds like giving whatever person who transform globes of invulnerability:
