Minthara may or may not be very grumpy with you when you return to the Shadow-cursed Land. However, should she be grumpy, you can win her affections back by donating all your trash ... I mean all your precious treasures to her through the barter interface. (You can later sell or use the stuff nomally.) I think the option to romance her only becomes available after defeating Ketheric, and the chat you can have with her after his demise can raise your approval enough to woe her. Iirc you need 30 approval and she will start the romance conversation. This can also happen in act 3. (I recently got it after defeating Orin.)
If you haven't picked any of of the main companion romances, Withers chides you and all your companions during their next attempt to reskill. (They have unique dialogue options if you make them chat with Withers, if you are curious.)
And yes, discovering the Grymforge from the Shadow-cursed Land is very unresponsive. The funniest inconsistency is if you proceed to the Myconids and then return to the forge. In my case it triggered the normal cut scene even though all the dwarves had long left. ^^
