On the 3rd or 4th night when your character is tired because he is starting to be affected by the parasite and has the dream where my guardian appears, after the dream my team wakes up and the message appears that my team was defeated and it is the end of my game.

Do you have Gale in your party and you did NOT feed him magic items?

I don't have him in the group, I killed him the first time he appeared.

Gale has a certain ... condition which, even after his death, can be fatal to everything inside a large radius. Gale even warns you - "If you strike me down I will become more powerful than you could possibly imagine." Or his Force Ghost says words to that effect
So, not a bug, but exactly as intended.

And now you know it was a misstake. ^_^

The only "safe" way to kill gale is:

I did an evil dark urge run,( which I abandoned halfway act 2), where I cut off Gale's hand and left him to die in his stone on the first contact. But I never had this explosion happening, even after many long rests.

Just as failing the "pull from rock" throw, what you did does not activate the Gale Go Boom! event.

Load last save before going to bed that fateful night, go to place where Gale's dead body is polluting the environment and revive him.
