I've noticed that at level 4, SH's portrait has been fixed. Which has lasted until level 5.
I have no idea if that was the issue, but I can confirm that after I did the "natural" level up from 3 to 4 last night, SH's portrait returned to normal - and that nobody else's glitched out. Besides her and Tav I had Gale and Lae'zel in the party when I did the level up.
Let's see what happens on the next one..
