I did the Save Mayrina quest and found the wand to bring her husband back, I decided not to give it to her because it breaks paladin oath and also because it seems profoundly fucked up, but I also didn't snap it in front of her face after telling her about it cause that also seems profoundly fucked up.

I decided to leave it at that, but on entering the Shadowfell I get the quest conclusion saying that I failed to save her. I know she's supposed to appear again in Act 3 in some capacity but does the game now think she's just standing by her dead husband's grave in the swamp forever and she won't show up? Or is the quest completion thing just wrong in saying I didn't save her?

Quest completion thing is wrong.

She'll still show up in Act 3 as long as she's alive.

Ah thanks for the heads up. I was leaning toward thinking it'd prob end up that way, but I also wouldn't be shocked if the narrative was she could never bring herself to actually leave and died in the swamp so wanted to be sure.

They must have changed that, when I played through (I think on patch 6 at the time) it just stayed there forever as an unfinished quest, as far as I can remember.

I think if I were designing it I'd let you find the wand, then talk to her and have a deception check to say you didn't find anything. Or maybe a persuasion check to say she shouldn't trust the hag. Then that could complete the quest

Yes, I have never made that choice, but when you raise the zombie-Connor, Mayrina decides to go to Baldur's Gate to find a way to truly bring him back.
Without raising Connor, I figure she has no incentive to travel to the city. So I think it is understandable she doesn't show up in Act 3.

Aye. But only if she survives Act 1.

Whether she is joined by Connor or not is subject to your choice at the end of the quest in Act 1 (Whether you use the Wand or not. Also if you use the Wand whether you keep it, and Connor, or not)

Thanks for confirming my suspicion that saving Connor is unrelated to Her Presence in The City.

I certainly didn't either give her the wand or snap it in front of her. Not sure if I spoke to her at all after finding it, but either way, she did turn up in act 3. If it's intentional (they might have just not thought about it), then it's probably because she's lost her farming life with Connor dying, and like everyone else she got drawn into the city. Doesn't explain how she got through the shadow lands so easily, but she did.
