Sneaking into the goblin camp - If you jump directly onto the log it breaks tumbling your character into the chasm for insta-death.
I think this is how I got the Shove Off-achievement. Tried to sneak across, Astarion missed the mark and destroyed the log. Following Tav pushed him in before I reloaded.
