It's affecting things behind various items in the UI such as character portraits, inventory, etc. If anything is behind them it will assume you're clicking that instead of the stupid portrait or item in the inventory or whatever. Can't pick up items easily or move items like barrels and boxes. Affects hovering over items to get the tooltip as well. This is basic easy to fix shit. It's 2026, don't sell the game in Steam claiming it's supported on a Mac if it's broken. Mac OS Tahoe.
