The game is very poorly optimized since patch 8, the game not being prioritized correctly by the macOS scheduler, causing massive fps drops.
Some have found that manually setting the game to high priority using terminal commands (renice) drastically improves performance, indicating a lack of native optimization by Larian.
Hotfix 30 were mostly aimed at people using M4 chips, and in OPs case they are only using M1 chip.
It is unfortunately very likely that Larian simply does not have the Mac programming experience/knowledge to deal with the problem.
