How on earth did you manage to miss Last Light Inn in Act 2? Or not look around the place once there? Anyways. once in Act 3 you can't go back to Act 2 so either you have to accept you can't romance Karlach at this point or you have to go back to a save in Act 2 (one before you went into the Shadowfell) and start over. All the romances get sorted out by the end of Act 2. I don't know of any mod that could help you out, possibly some consol commands if you are using a PC. but the only thing that would do is set her flags to be already your romance partner so you wouldn't get any of the dialogue or interactions you missed out on.
