Title is pretty much my question. I donated via PayPal and don't know how to get the digital rewards once the game comes out. I got a transaction code, is this everything needed?

You will get a mail with further information I guess. Your paypal account is linked to an e-mail of you so the devs know how to contact you after the campaign.
