1. What to do with the house/letter of Romeo next to the merchatn's area?
2. I found a sapphire key, where cna I use it?

As for the Saphire key...

I have only just gotten out of Act I, and have found one or two of those keys. In those instances, they were usually special keys for special doors that couldn't be opened using regular keys. Maybe you haven't found the special door yet? Usually I needed them to get through doors that let me get further in the game.

IIRC, that key is for a locked chest in or around the Romeo house.

Or is it just a house with a letter and a chest?

There is no quest involved with the house or chest; it is just a reference to Romeo and Juliet. There are also other references around to games, books, people, websites (written in runes) etc;
