I have completed all trainer quests, except for the second one given by Kenneth. As I didn't encounter a conversation option with him, I decided to continue on playing a bit and to return to him later. But still, nothing about a quest!

I've played the game a number of times before, so I didn't mind getting spoiled and read on the internet this quest involves killing Kali. I have already done so. Does this have anything to do with the quest not appearing in-game/with Kenneth?

Did you talk to Sassan and see if she has a conversation option about finishing the first upgrade?

Are the first or second quests listed in the open/closed quest log, with an appropriate description?

There should not be a problem killing Kali before getting the quest; she drops the Demon Wars book even if you chose Hermosa. Did you read the book yourself? It gives you 5 skill points (3 in DKS), but will cause the failure of the second skill platform upgrade quest if you picked Kenneth.

I read the book, as it came in my possession before I was given the quest by Kenneth.

I had already finished the game when it came out, but now I started playing again because I want the Xbox 360 Achievements.

The only thing I can do now, is start over, right?

You can read the book (without failing a quest) if you pick Hermosa as your trainer. Similarly, if you pick Barbatos as your alchemist, you can drink a Potion of Wisdom (+5 Intelligence) that Allan wants for one of his quests.

If you still need the achievement for fully upgrading the battle tower, you will have to either reload a save before reading the book or start over with another character.
