I've had this happen twice now, once during the final boss and once just now in a random encounter outside of Cyseal. One of my characters died and I revived him, but the character model was invisible and he was just a piece of floating armor. No spell or attack animations either as far as I can tell. This didn't go away after the battle ended, or after a teleportation. Not sure if reloading would have worked, I ended up replaing the battle without dying.
