I've just played around Sacred Stone in the EE and came about Bruthor the orc in the immaculate jail.
Well, aside from the strange issue of him attacking me when I open his door while the guards are still alive, he also won't recognize if I free him via teleport, despite him asking me to do so (after I've killed the guards, obviously - they didn't react very friendly to me opening an empty cell).

It seems that bug has been around since before the EE (although I don't remember what I did in the old version).

Any ideas?

He does make a comment when teleported out that stretching his limbs feels almost as good as bashing in the skulls of those who imprisoned him, though his conversation doesn't change if you talk to him. He will then attack the guards, if they are still alive. If you don't try to help the orc, your party shouldn't get pulled into the fight.

Ah yes, I noticed that attacks the guards when I teleport him out.
He hasn't much of a chance, though. Also, strangely, when I tried to heal him, he became hostile and the guards allied with me.
The prison is a bit strange, tbh. - it seems almost impossible to help the prisoners without suffering a pretty strong reputation hit (-2) or letting them meet their doom by hands of the guards.
Also they have a tendency to turn against me, similarly to the orc, if I try to help them against the guards.

Strange; healing him should turn the guards hostile towards you.

Nope, when I enter combat by healing him (because he's going to lose on his own) it's always him getting hostile and the guards being "green".
I think it's the same with the other prisoners, even though it was me who freed them.

That happened to me once using Boulder Bash on a couple of the guards, but I think Bruthor was just barely inside the area of effect, so between neutral and friendly units, the neutral one went hostile.

The guards were all dead by the time I freed the other prisoners, so they all just ran away.

It is possible to free and effectively save Bruthor and all (or some of) the other inmates without loosing reputation.

First, you should not attack and fight the immaculate jailers in plain sight of the inmates, because Bruthor or the other inmates will become hostile in this case. Same thing as launching an attack on Dreksis and his goblins in the Luculla mines during the quest "Goblin Trouble", the imp Mittix will become hostile in this case.
Second, you will loose reputation only, if a jailor sees you, while you open a jail cell door.

Instead, wait for the right moment and enter Bruthor's cell unseen. If a jailor passes by, he will lock the cell door again, but no repution is lost and no fight starts. Then put a teleportation pyramid in front of Bruthor. He is like an uneducated playful child and uses the pyramid, which teleports him to the other pyramid, which was in my case in the immaculate academy. And no - no cruel jokes with Bruthor, at that time no immaculate was alive anymore in Sacred Stone and I had the prison cell door key of the Enlightened Teacher. Pick up the pyramid and follow him, Bruthor comments something about freedom and his desire to walk around, and some time later he had left that place and was nowhere to be seen.

Now attack and kill all immaculate jailors beside Bruthor's cell, they don't deserve anything better, do they?
Then you may release the other inmates, who will run away immediately. But talk to them before, and in my case, the immaculate prisoner, the woman who was imprisoned after her husband showed mercy with some lost ones, didn't make it to freedom. Jahan teleported her out of her cell, and as a fanatic supporter of the Conduit, she preferred to attack my party and justice was served. It's an RPG after all, deep and with many details.
