A few people ran into a crash problem entering Verdistis (and the council hall) due to a corrupt world file in the save folder, but without the additional problems you have. see;

Game crashes when I try to return to VerdistisThe first time you enter the church there is a cutscene triggered. If this is the cause of that particular problem, it may help to run the configuration and temporarily switch to software mode and lower the resolution (if possible).
Does the graveyard portal crash when you trigger the map, or after you select a destination? If the latter, does it just crash teleporting to Verdistis or the council hall?
Where is Zandalor when you try to talk to him, in the castle cellar (accessed through the sewer), by the inn with the other marked one or in the council hall? If the latter, the problem with the Verdistis crash is not due to a corrupt world file. If the either of the former locations, is the crash when you actually try to talk to Zandalor, or one of the cutscenes as you approach?
Re-installing the game and trying another PC but still having the same issues implies the problem is in the save folder (though it is not impossible you could have the same unreported error happening on install). Do you have any recent saves that are working? If not, can you start a new game, use
iZakaroN's SaveEditor (or from
Kiya's site, with description) to boost the character, then run to the church and Verdistis?
