There is a search function in the forum, in the text menu bar near the top of each forum page (white text on a blue background), though I don't think that would have helped much for the second question, at least.
Random errors are difficult to track down...
Do you have any other DirectX games that you have played recently? Similar problems in other games (or resource intensive programs) would point to a system issue, while DD specific problems would imply a conflict between the game and system somewhere.

BattleFields N00b ?Windows uses a cache for shortcut icons, which has a tendency to become corrupt. Resolution switching with low system memory (exiting a game) may increase the chance of Windows having a problem. Alternately, since the game is full screen, corrupt icons are only going to show up when you exit (or alt-tab out of the game). That said, I don't think DD should be any more of a problem than any other game in this regard.
Tweak UI is one of the
Microsoft PowerToys for Windows XP, and contains a function to recreate the icon cache (under the Repair section).
