1. At toll house

I lured Flind pack from the cave area to Anders toll house and let the 2 groups kill each other off.

So I didn't have any conversation with Anders. However after I use speak with the dead on Bedroll(the dead guy inside toll house), Lae'zel mention "We find this one-horned tiefling and collect our prize." Which we should have no information on.

2. At mountain pass

I killed the ginthyanki patrols and obtain the slate. If I use the slate before talking to Laezel, the "they try to kill you ..." part will be missing from later conversation with Laezel.

I had a similar issue (without killing Anders first). If you speak with the dead refugee, Lae'zel gets the exclamation point over her head. Interesting thing too is at this point we know they are aligned with fiends, yet if you then talk to Anders it doesn't come up and isn't noted in the quest log. But talk to Anders first and then talk with the dead, it'll add a note to your quest log who they're aligned with.
