Yeah, I'd say only one of the options in that set is actually a come-on. Two are neutral, the last leans more dismissive (and might make no sense depending on the path you take through the conversation, I'm assuming that's a bug or an oversight).
I'd like to see a more nuanced approach than just high approval = green light for romance. I'd like to see dialogue choices make a difference, and I'd like it if some actions - for instance, letting Astarion snack on you - had more significance than just adding to the approval count. But this is all complicated stuff; maybe Larian doesn't want to tie up resources in it.
