Please email supportdos@larian.com, with a description of the problem and the report.zip file generated by the D:OS support tool. The report will contain your saves (you can delete some if file size is an issue), so with a couple of the working saves, and some of the ones which have this issue, it should help Larian track down the problem.
Include your save just before reaching level 4, and Larian can check if it is repeatable, at least, and then hopefully either fix it or prevent it from reoccurring.
